Please complete and submit your request for pool tags - All areas must be completed.  Incomplete request will not be processed.

Pool tags can be mailed to resident for a $5.00 handling fee that must be submitted with your pool tag request via US Mail. (Mail request can not be submitted online)  All other tags will be available to pick up at the pool party. Association Dues and Deed Restrictions issues must be current in order to receive 2017 pool tags. Please note that children under ten (10) must be accompanied by someone sixteen (16) years or older. Children under ten (10) will not be issued pool tags.
Pool tags will be required for entry to the pool area effective June 3, 2017 - regular pool season.  Pool tag must be shown to Life Guard when entering the pool gate.  

Pool tags can be requested via email by sending an email to mccia@memorialchasecia.com and including the required information, incomplete request will not be processed.   Association dues and deed restriction issues must be current for pool tag requst to be processed.
